In order to properly manage your account we require the following before we can help you.

~ We prefer that you have your own authority. If you don't have it, we can still be of service to you but we will need to have the company you are leased through complete the Service and Agreement & Limited Power of Attorney forms.

~ A copy of your authority.

~ Insurance Certificate listing D & R Dispatch Services, LLC as a Certificate Holder. Our address is on the Contact Us page. We recommend you have $1 million in liability and $100,000 in cargo. (Many brokers require this amount)

~ A properly signed W-9 form

~ You must be a subscriber to The Internet Truckstop, and in good standing with their service. (This is for truck posting capabilities) For additional information on how to become a subscriber to The Internet Truckstop, click on their logo below.

~ All forms need to be completed either electronically or downloaded and either mailed or faxed to us.
